Location: Shelton, Connecticut

Schedule / Job Type: Contract | Days | 13-Week Assignment

Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
Administer therapeutic exercises, manual therapy, and other physical therapy interventions to promote patient recovery
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Educate patients and family members on home exercise programs and injury prevention strategies

Required Qualifications

Active Physical Therapist (PT) license in the state of Connecticut
Graduate of an accredited Physical Therapy program
Experience working in a skilled nursing facility (SNF) setting
Current CPR/BLS certification

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with electronic medical records (EMR) systems used in SNF environments
