**EMR:** Meditech (HospiceSoft/Careficient experience preferred)

### Requirements


✔ Active Utah RN License or Compact RN License
✔ Minimum 3 Years of RN Experience
✔ Hospice, Home Health, or Palliative Care Experience Preferred
✔ Current BLS Certification
✔ Valid Driver's License & Reliable Transportation
✔ Strong Assessment, Communication, and Care Coordination Skills


### Job Responsibilities


• Perform comprehensive patient assessments and develop individualized care plans
• Provide pain and symptom management for hospice patients
• Educate and support patients and families regarding end-of-life care
• Collaborate with physicians, social workers, chaplains, and interdisciplinary teams
• Maintain accurate and timely documentation
• Participate in on-call rotation and weekend coverage as needed


### Additional Details


On-Call: 1–3 shifts per week, including approximately 3 weekends during the contract
Holiday Coverage Required
⏱ Must be able to respond within 45 minutes when on call
