What You'll Do Deliver therapeutic interventions established by the supervising Physical Therapist. Assist patients with strengthening, balance, gait training, transfers, and mobility exercises. Reinforce home exercise programs and educate caregivers. Monitor patient progress and communicate updates to the supervising PT. Collaborate with physicians and the interdisciplinary care team. Complete accurate, timely documentation. What We're Looking For Active Minnesota Licensed Physical Therapist Assistant (PTA) license. Compassionate, patient-centered approach to care. Strong communication and organizational skills. Home health experience is helpful but not required. Valid driver's license and reliable transportation. About Home Health Care, Inc.
