Therapist - Physical Therapist

Job Description

Job Summary:
Provide comprehensive physical therapy services to adult home health patients in their homes, focusing on improving mobility, strength, and overall well-being. This role involves patient assessments, developing treatment plans, and ensuring documentation is completed accurately and promptly.

Responsibilities:

• Evaluate patients' physical conditions and develop customized treatment plans.
• Conduct therapy sessions and monitor patient progress throughout the course of care.
• Document all clinical interactions accurately using EMR systems within 24 hours of service.
• Travel to patient homes up to 75 miles from the clinic to deliver therapy services.
• Collaborate with team members during periodic office meetings and maintain compliance with agency protocols.
• Adjust treatment plans based on patient progress and clinical judgment.

Qualifications:

• Hold a current, unrestricted license as a Physical Therapist.
• Possess at least 1 year of recent adult home health experience.
• Have reliable transportation, a valid driver's license, and current liability insurance.
• Be proficient in EMR systems such as PointCare or HCHB used within the past 6 months.
• Demonstrate excellent documentation, communication, and time management skills.
• Willing to travel up to 75 miles and accept patients regardless of distance.
