Registered Nurse - Case Manager

Role: RN-Case Manager providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses. Responsibilities: Conduct weekly in-home nursing visits, perform physical assessments, ensure home safety, manage medications, oversee caregivers, and coordinate care plans. Collaborate with regional staff to improve services, participate in staffing and training, review clinical documentation, and contribute to quality improvement initiatives. Support client health, quality of life, and safety through personalized care and case management. Qualifications: State-licensed RN, BSN preferred, with at least two years of nursing experience including one in home care. CPR certification required. Supervisory or case management experience is a plus. Skills: Strong clinical assessment, care planning, team collaboration, documentation, and adherence to policies. Ability to work independently and manage client needs effectively.
