Pharmacist (Clinical Specialist)

Pharmacy Technician

Major Duties to include but not limited to the following:

Clinical Practice

  • Maintains a clinical practice through provision of patient care during new patient and/or follow-up visits. Coordinates care for patients with other healthcare providers.
  • Provides and coordinates medication management services for patients in the designated clinic(s) according a broad scope of practice focused on chronic disease state management. This shall include:
  • Initiating, discontinuing and/or changing medications through the electronic medical record
  • Ordering labs and other tests to help determine efficacy and toxicity of medication therapy
  • Managing care across the continuum of the health care system
  • Referring patients to appropriate health care providers
  • Performing physical assessments as needed
  • Performing patient triage functions as needed
  • Documents all encounters in the patient's clinical record.
  • Demonstrates competence in differentiating individual patient pharmaceutical care needs based on patient age from adult (18-64) through geriatric (65+)
  • Renews and orders medications per Scope of Practice with peer review.
  • Evaluates orders for appropriateness and drug interactions, allergies and adverse reactions, and duplicate therapy.
  • Provides appropriate patient consultation for all new prescriptions. Inquires about side effects and patient questions for refill prescriptions. Evaluates patient's ability to understand instructions. Provides verbal and written instructions to promote patient medication adherence as appropriate. Documents all elements of patient consultation.
  • Accountable for comprehensive medication management according to procedure.
  • Maintains an in-depth clinical knowledge of chronic disease state management.
  • Provides direct patient care for comprehensive medication management which includes chronic disease state management.

Distributive Functions

  • Reviews service connection for appropriate costs and payment and applies VA local standards for appropriate dispensing per departmental needs.
  • Supervises technical staff to maintain efficient and effective operations per departmental needs.
  • Checks and approves items prepared by pharmacy technicians per departmental needs.

Clinical Education and Scholarly Activities

  • Serves as a preceptor and shares responsibility for training, guidance and evaluation of learners including students and residents.
  • Assists the Residency Program Directors in the direction and coordination of the program.
  • Serve as an intern preceptor for School of Pharmacy Pharm.D. students during rotations.

Administrative Support

  • Independently manages comprehensive medication management services for ambulatory patients.
  • Manages quality improvement programs for comprehensive medication management services and coordinates activities with other services.
  • Evaluates pharmacy related performance measures for a designated group of patients and initiates program changes to achieve measures.

Customer Service and Teamwork

  • Maintains excellent working relationship with department and organization staff.
  • Is courteous and professional at all times.

Work Schedule: 8:00am - 4:30pm, Monday - Friday

Pay: Competitive salary and regular salary increases.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $625.00

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Telework: AD-HOC Virtual: This position is currently operating under an extension to the return-to-office requirement. That extension can be withdrawn at any time, at which point the employee would be required to return to the office.
