As a valued member of our team, we are seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Build strong relationships with the facility and maintain a patient-centered approach

• Provide preventative care to residents in skilled nursing facilities

• Collaborate with other healthcare professionals to ensure comprehensive care

• Utilize remote patient monitoring, chronic care management, and collaborative care management to minimize hospitalizations

• Achieve high-quality care standards and improve CMS ratings

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and Documentation review

• Excellent communication, collaboration, and teamwork skills
