Responsibilities
- Perform PT evaluations for residents with mobility, strength, balance, and functional needs; establish individualized treatment plans.
- Implement evidence-based interventions to improve gait, transfers, activities of daily living, and overall mobility.
- Reassess resident progress regularly, adjust goals, and document outcomes in required health records.
- Collaborate with an interdisciplinary team (nurses, therapy aides, and other therapists) to coordinate care and optimize outcomes.
- Provide patient and family education, discharge planning input, and home exercise programs to support continued progress after therapy.
- Ensure adherence to safety, infection control, and privacy standards while delivering high-quality care.

Requirements
- Active Physical Therapist license in Delaware
- Doctor of Physical Therapy (DPT) degree or equivalent
- Relevant experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to work in a long-term care setting and participate in a travel assignment
- Strong documentation, communication, and teamwork skills
- Comfort with a 35-hour guaranteed workweek and 08:00-16:30 shift schedule
