Key Responsibilities\n

• Conduct thorough assessments of clients' mental health needs, ensuring all evaluations, group notes, and documentation are completed in a timely manner as per regulatory standards.

• Collaborate effectively with a multidisciplinary team of professionals to create comprehensive discharge plans and assess client readiness for transition.

• Facilitate psychoeducational and process groups, addressing topics relevant to our clients while promoting interactive participation.

• Develop individualized treatment plans and actively participate in regular treatment planning meetings, continuously reviewing and adjusting plans to reflect client progress.

• Monitor and assess safety within the client's environment, promptly reporting any identified risk factors or changes in condition that may threaten their well-being.

• Utilize evidence-based interventions in one-on-one and group counseling sessions, ensuring your therapeutic methods are backed by the latest research and best practices.
