Job Description
Job Description LPN shall act as an active member of the interdisciplinary team conducting at least, but not necessarily limited to; assessment and screening, participation in team conferences, case management, discharge planning, intake and referral and other clinical duties. The LPN will provide a valid and current Louisiana license prior to hire.
Responsibilities and Duties
Works effectively in a team atmosphere.
Infection Control
Interacts in a respectful manner when working with patients and families.
Provides assistance to individuals in a manner that takes into account their special needs or age related needs.
Demonstrates knowledge of Nursing Standards of Care.
Demonstrates knowledge of Patient and Family Education.
Identifies problems and needs of clients and develops a nursing plan designed to meet clients’ needs.
Assist Medical Director with verbal and written physician orders.
Flexibility
High level of patience, empathy, and compassion
Excellent observation skills with high attention to detail
Excellent oral and written communication skills
