Summary We are looking for a clinically competent, licensed practical nurse (LPN) who delivers care as a team member to acutely ill patients under the direction of a registered nurse (RN). The role requires proficiency in the nursing process: assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation. The LPN must effectively interact with patients, significant others, and health‑team members while maintaining standards of professional nursing. Essential Functions Provides care to acutely ill patients; the LPN is knowledgeable of care goals and objectives and employs a team approach. Follows the plans of care developed by the RN and contributes data to the RN in developing the plan. Maintains effective communication with the primary RN, physician, patient, family, and other health‑care team members. Seeks direction in patient‑care delivery and reports current developments and problems to the designated RN, using the proper communication system. Assists in assessment of patient condition, reactions, and in planning individual care. Reviews transfer and admission information, daily progress and HCT notes. Assists, as necessary, in the initial nursing assessment of patient (history & physical). Provides information for establishing and re‑evaluating nursing goals and nursing care plans. Documents and reports pertinent information, including medications (aware of potential side effects) and uses nursing care plans as guides. Orders, per physician; requests, per therapy (include response of patient). Measures and records vital signs, I & O’s, weights, and administers treatments, including sterile procedures. Assists other disciplines in evaluating and implementing specific goals for the patient (physical, occupational, speech, recreational therapy, and dietary). Participates in patient education (and family education, when indicated). ADL’s Skin care Medication Disease process (CVA, diabetes, UTI, etc.) Reviews and implements physician’s orders under the direction of the primary RN. Administers medication, IV’s if certified in‑house. Documents response and/or side effects. Knows unit‑dose procedures. Renders basic nursing care, accurately charting the same. Provides care for patients in accordance with nursing policy and procedure, unit‑specific competencies and standards of care. Demonstrates ability to handle and assist in emergency situations promptly, precisely, and professionally. Compassionately provides personal patient care, acknowledging physiological and psychosocial needs. Administers medications per nursing policy and procedure. Observes and documents patient’s response to pertinent medications. Demonstrates appropriate technical skills when administering treatments and procedures per physician orders and nursing policy. Exhibits interpersonal skills that promote a positive department functioning. Communicates effectively with peers, health‑team members, management, and physicians. Maintains confidentiality of all patient‑hospital or physician‑related information. Interacts effectively with patients’ families and/or significant others, greeting patients courteously and respectfully. Adapts to changes professionally. Supports problem‑solving approaches to both department and patient needs. Follows through on problems that may compromise patient care by using the appropriate chain of command. Accurately and concisely processes information. Communicates appropriate information to the primary RN as needed. Communicates appropriate information to physicians and other health‑team members timely. Demonstrates accurate and timely documentation, neatly and legibly, per form guidelines. Accurately charges for patient supplies. Maintains a safe and sanitary work environment. Ensures a clean, neat, and safe environment per department policies. Stores supplies and equipment necessary for patient care in an organized manner. Uses equipment safely, following product information and policy manuals. Obtains, returns, or cleans/dispenses equipment and supplies appropriately. Collaborates with hospital personnel and physicians. Communicates daily regarding patient needs, nursing assessments, and recommendations. Collaborates with other nurses and health‑team members to assure patient well‑being. Adheres to hospital/nursing policy. Observes guidelines concerning absences, overtime, and tardiness. Dresses according to CWH dress code for nursing; is conscientious about personal hygiene. Complies with infection‑control policy. Assumes responsibility for self‑evaluation and improvement. Maintains 100 % attendance at mandatory in‑services, including CPR certification and annual safety in‑services. Consistently advocates and functions within the guidelines of the Washington Nurse Practice Act and keeps current with legal aspects of nursing. Demonstrate standards of behavior and adhere to the code of conduct in all aspects of job performance at all times. Qualifications Graduate of an approved LPN program or an RN program. Current nursing license in the state of Washington. Completion of hospital orientation program. Must be currently certified in CPR and recertify annually. Desired Not specified.
