The LPN is responsible for administering medications, monitoring resident health, assisting with care plans, and ensuring high-quality care in accordance with facility policies and state regulations.### Key Responsibilities* Provide direct nursing care to residents in accordance with care plans and physician orders.* Administer medications and treatments as prescribed.* Monitor residents’ vital signs, conditions, and responses to treatment.* Report changes in resident condition to the Registered Nurse or physician.* Assist with the development and implementation of individualized care plans.* Supervise and support Certified Nursing Assistants (CNAs).* Maintain accurate and timely medical documentation.* Ensure compliance with infection control and safety standards.* Respond to resident needs and emergencies.* Participate in shift reports and interdisciplinary team meetings.* Provide compassionate care while maintaining resident dignity and privacy.## Qualifications### Qualifications* Current **Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)** license in the state of employment.* Graduate of an accredited practical nursing program.* **Experience in long-term care or skilled nursing preferred.*** Current **CPR certification** required.### Skills and Abilities* Strong clinical and patient care skills* Medication administration knowledge* Excellent communication and teamwork skills* Ability to monitor and respond to changes in resident condition* Strong documentation and organizational skills #J-18808-Ljbffr
