Social Worker Seattle Full Time Variable

Overview Northwest Kidney Centers seeks a Social Worker to support the Social Work department in a variable hours position (e.g., 8 am–4:30 pm, 12 pm–8 pm). The role is full‑time (1.0 FTE), Monday‑Friday, variable hours, on‑site. Hourly rate: $36.05–$46.95–$57.87. The Social Worker reports to the Social Services Manager and is responsible for social work intake, patient advocacy, coordinating services with physicians, NKC employees, patients, families, and other organizations. The position promotes patient voice, facilitates communication through interpreter services for non‑English fluent patients, and educates patients on rights and the NKC grievance process. It includes patient education, counseling, and, when required, relief or project duties. Duties Member of the interdisciplinary team, independently conducts comprehensive assessments and unstable care plans, and develops plans of care. Administers the KDQOL‑36 tool with patients to identify areas of focus for improving quality of life. Conducts depression screening, monitors mental health needs, offers emotional support, brief therapy, and referrals to outside agencies. Screens for palliative care and refers to appropriate providers, including the NKC Renal Supportive Care team. Coordinates casework duties; performs psychosocial assessments, counseling, education, suicide assessments and management, and documents related activities. Serves as a resource for patients and staff regarding transportation to dialysis and related appointments and advocates with transportation providers. Problem‑solves with patients on employment continuation or resumption; coordinates with vocational rehabilitation agencies. Coordinates social services between patients, families, physicians, NKC chaplain, NKC employees, nursing homes, area hospitals, and other organizations. Member of the QA/PI process, reports KDQOL survey response rates, analyzes trends, and develops action plans to address worsening trends. Participates in unit huddles and assists in developing plans to improve patient outcomes. Works with the IDT to implement unit quality initiatives related to transplant, hemodialysis access placement, and hospitalizations. Point person on the IDT for tracking transplant status and collaborates with Project Access Northwest to help eligible patients receive dental clearance for transplant. Provides education to NKC employees on social work topics such as suicide management. Serves as a community resource for the treatment teams, acting as a patient advocate and educational resource to NKC staff. Performs relief, project, and committee assignments, including coordinating special projects and developing teaching materials for staff. Leads or participates in meetings with NKC employees and performs ad‑hoc, standing, and committee responsibilities. Keeps informed of current professional standards, keeping department policies and practices up to date. Performs other duties as required, including teaching classes related to pre‑dialysis patients such as Next Step and Choices classes. Requirements Must hold a Master of Social Work (MSW) degree from a school accredited by the Council of Social Work Education. Must hold a Washington State Independent Clinical (LICSW) or Advanced Social Worker (LASW) license, or an Independent Clinical Associate or Advanced Associate license. No previous experience required; one year of progressively responsible clinical experience in a medical setting is preferred. Must possess a valid Washington State Driver's License and car insurance.
