Description

• Assess patients in both inpatient and outpatient environments and create personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Educate patients and their families on therapeutic goals, discharge planning, and home exercise programs
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in compliance with facility and legal standards
• Engage in multidisciplinary care team rounds and collaborative conferences to foster seamless communication

Education

• Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
