Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.)Completes reassessments including OASIS assessments at appropriate time points to determine level of functioning. Updates and revises the plan of care in consultation with the physician and interdisciplinary team members.Provides therapeutic treatments and evaluation of equipment needs as applicable. Prepares clinical and progress notes that support skilled interventions and assesses outcomes of care. Plans for discharge of patient.Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings and case conferences.Coordinates the Occupational Therapy Assistants and Home Health Aides and services including completion of required supervisory visit documentation. Participates in training and support of all team members, volunteers and students as needed.Carries out treatment plan effectively and modifies to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care. Utilizes testing results and develops updated plan of care based on principles outlined in appropriate evidence based guidelines.QualificationsEducationBachelors Degree, Occupational Therapy (Required) orMasters Degree, Occupational Therapy (Required) orDoctoral Degree, Occupational Therapy (Required)Experience1 year Occupational Therapy experience (Required)Skills and AbilitiesDemonstrates the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care.Demonstrates knowledge of the principles of growth and development throughout the life span, be able to assess patients' needs and status by analyzing evaluated data, and be able to interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting.Flexible with daily scheduling and may be required to work various patient care settings.Licenses and CertificationsLicense Occupational Therapy (OT) - Virginia Department of Health Professions (VDHP) Upon Hire(Required) andLicense Occupational Therapy (OTR) - National Board for Certification in Occupational Therapy Inc (NBCOT) Upon Hire(Required) andCPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the jobValid Drivers License RequiredTo learn more about being a team member with Riverside Health System visit us at #J-18808-Ljbffr
