Job Overview The Senior Manager of Hospital Outreach leads a team of Dialysis Transition Managers (DTMs). These individuals conduct outreach to hospitals to improve the transition of care experience for renal patients moving from hospital to outpatient dialysis. The Senior Manager serves as a strategic thought partner and operational leader, enabling the evolution of our DTM program. In this role you will manage and develop a high‑performing team of DTMs and architect high‑impact projects, pilot programs and strategic initiatives designed to innovate the transition of care experience for renal patients. Responsibilities Design and execute complex, multi‑stakeholder strategic initiatives. Elevate the coaching and development of your team to identify growth opportunities and generate actionable insights. Collaborate across clinical, operational and technical business units. Communicate compelling, data‑driven recommendations to senior executives. Act as strategic owner for key programmatic workstreams such as launching new outreach pilots, process improvements and research projects from concept through execution and scaling. Leverage a hypothesis‑driven approach to identify, validate and quantify growth and operational excellence opportunities within the hospital ecosystem. Distill complex business problems into cohesive, impactful stories and present to executive audiences. Develop infrastructure around data, tools and processes to create sustained programmatic capabilities and improve program metrics. Continuously push thinking and find opportunities for innovation and growth. Coat and develop DTMs to improve the skills necessary for success. Create a special place by fostering belonging, psychological safety and continuous learning. Manage relationships with senior leaders and peers and influence stakeholders through effective communication and strong interpersonal skills. Align with clinic leaders on local growth strategies. Create and manage project plans that drive accountability for cross‑functional initiatives; allocate team resources to ensure all timelines and deliverables are met consistently. Be available for up to 30% travel (typical 10–20%). Minimum Qualifications 7–10 years of total work experience in progressive roles, preferably in strategy development, project management or complex operations management. Proven track record of leading and developing high‑performing teams. Experience driving transformational change and managing special projects from concept through implementation with tangible impact on operating performance. High degree of data literacy and analytical problem‑solving skills; ability to translate raw data into strategic action. Experience or degree in a healthcare‑related field strongly preferred. Salary The salary range for this role is $85,000 – $135,000 per year. Compensation may vary based on qualifications, skills, competencies and experience.
