Job Summary Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini‑mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a timely fashion compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine the reason for the absence and when they plan to return to the program. Responsibilities Design a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments. Review and update treatment plans every 30 days. Perform a mini‑mental status exam and psychosocial assessment on all new patients. Provide group and individual therapy and prepare timely documentation in compliance with company, state, and federal guidelines. Perform related duties as required. Perform other duties as assigned. Qualifications Education: Master’s degree in Social Work or related field. Experience: 2 years of related experience. Work experience may not substitute for minimum education requirement. Licensure/Certification: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy; CPR. Salary: $36.95 - $57.27 per hour (individual wages determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience).
