AMGSpecialtyHospital -Muncie Campus is seeking aFull Time Case Manager (RN, LPN). TheCase Manager's primary function is to make readily available to the patient,their families and/or significant others, special services in order tofacilitate adjustment of these individual's continuity of care postdischarge. The Case Manager oversees the effective coordination ofadmission and discharges planning, case communications, conferencing, insurancerelations and assurance through managed care systems, and verbal and written reporting.Assures regulations regarding patient rights are fulfilled. Allcandidates must have a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Indiana License in good standing.

Job Requirements

• CurrentRegistered Nurse (RN) Indiana License or Licensed Practical Nurse (LPN) IndianaLicense in good standing.
• CurrentBLS certification.
• CurrentALS certification.
• Minimumof 1-3 years of related work experience as a Case Manager.
• Abilityto read and communicate effectively in English.
