Referral Intake Specialist - FT Evening Shift

Patient Support Specialist

This is an onsite, 40-hour-per-week position, with shifts scheduled between 12:00 p.m. and 10:30 p.m. Team members participate in an every-third-weekend rotation and work one to two holidays per year on a rotating basis. In this role, you will support patients, families, referral sources, and care teams throughout Ohio's Hospice service areas. The ideal candidate is compassionate, service-focused, and comfortable working in a fast-paced environment.

Primary Responsibilities

• Respond to incoming calls and inquiries with professionalism, empathy, and excellent customer service.
• Collect and document accurate patient and referral information.
• Explain hospice services clearly and respond to questions from patients, families, providers, and community partners.
• Maintain complete, accurate, and timely records of all interactions.
• Collaborate with team members and support other departments as needed.
• Participate in quality-improvement initiatives that strengthen patient and family care.
• Assist with onboarding and training new team members when assigned.

Qualifications

• High school diploma or GED required.
• Strong computer and data-entry skills.
• Excellent communication and customer-service abilities.
• Valid driver's license and a safe driving record.
• Proof of state-required automobile insurance.
• Ability to travel in varying weather conditions when necessary.
• At least two years of customer-service experience preferred.
• Ability to successfully complete a criminal background check and 10-panel drug screening.
