Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
Provide direct occupational therapy services to patients to restore, maintain, or improve functional abilities
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate and optimize patient care
Educate patients and their families on therapeutic techniques and strategies to support recovery and independence

Required Qualifications

Graduate of an accredited Occupational Therapy program
Current and valid Occupational Therapist licensure in the state of Hawaii (or eligibility to obtain)
Current NBCOT certification
Experience working in a skilled nursing facility (SNF) setting
Strong documentation and communication skills

Preferred Experience / Skills

Previous travel or contract therapy experience
Familiarity with electronic medical records (EMR) systems used in SNF environments
