Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility Location: Highland Chateau Health + Rehabilitation Department: Nursing Reports To: Director of Nursing Job Type: Full Time, Part Time, PRN Shift: Days, Evenings, Overnights Responsibilities Provide direct nursing care to residents in accordance with their care plans Administer medications and treatments as prescribed by physicians Monitor residents’ conditions and report any changes to the RN or physician Document all nursing interventions and resident responses accurately and promptly Assist with assessments, admissions, discharges, and transfers of residents Collaborate with CNAs and other staff to ensure the highest quality of care Maintain a clean, safe, and supportive environment for residents Provide education to residents and families regarding health conditions and care needs Uphold all state, federal, and facility regulations and standards of care Participate in ongoing training and quality improvement initiatives Qualifications Current and valid LPN license in the state of Minnesota Graduation from an accredited practical nursing program CPR/BLS certification Long-term care or skilled nursing experience preferred Strong clinical, communication, and critical thinking skills Compassionate, reliable, and team-oriented attitude Benefits Competitive pay and shift differentials Health, dental, and vision insurance Paid time off and holidays Referral bonus Sign on bonus: $4,000 (Full Time)/ $2,000 (Part Time) Tuition assistance and career development support Employee wellness and recognition programs #J-18808-Ljbffr
