Job Description

This position involves providing therapy as a Physical Therapist Assistant (PTA) in a home health setting, conducting home visits and participating in periodic office meetings. Candidates must possess reliable transportation, a current driver's license, and agency-required liability insurance.

Patient Care Details

• Average Number of Patients per Day: 5-6
• Weekly Points Requirement: 30 points per week; weekly activities, including documentation and travel, are assigned different point values.
• Types of Cases: Start of Care and Follow-Up visits
• Expected Travel Distance: Up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.

Experience Requirements

• Years of Experience Required: Minimum of 1 full year of adult home health experience. First-time applicants are not accepted; recent home health experience is mandatory.
• Certifications Required: CPR and BLS certifications.

Contract Details

• Contract Duration: Typically 13 weeks
• Weekly Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend Rotation: Varies by location and profession, to be discussed in the interview.
• On-Call Requirement: Varies by job/location; please discuss with the manager during the interview.

Travel and Documentation Expectations

• Contractors must accept all patients on their schedule, regardless of the distance from previous appointments.
• Documentation must be completed within 24 hours of the patient appointment to ensure efficient billing and patient follow-up.
• There are no guaranteed hours; contractors must meet productivity points to support their financial needs.
• Consultants will have the opportunity to float to a nearby clinic if there is low patient census.

Location

The candidate will be covering the following territories: Norton, Attleboro, Mansfield, Plainville, North Attleboro, Wrentham, Foxboro, and Seekonk.

Required Qualifications

• Must have proficiency in OASIS and experience with HomeCare HomeBase EMR within the last 6 months; retraining may be required if not.
• Must have a valid license in hand to apply; no exceptions.
• Must not currently be a direct employee of any organization affiliated with this position.

Additional Information

Applicants should confirm their comfort with the demands of this home health assignment, including the daily responsibility for driving to patient homes and attending weekly meetings, as well as the ability to handle longer travel times between appointments.
