SUMMARY:
Under the general supervision of the Director, Community Health Institute, establishes and maintains effective relationships with community representatives and organizations to design, promote and deliver health education, screening, and intervention programs, especially as related to vulnerable populations. Identify community needs, prepare proposals and coordinate programs to meet same.

RESPONSIBILITIES:
Consistently applies the shared values of compassion, accountability, respect, and excellence in improving the health status of the people of the region through the provision of customer-friendly, geographically accessible and high-value services within the context of a comprehensive, integrated, academic health system. Is responsible for knowing and acting in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct.
Under direction of the Director, Community Health Institute, participates in community health assessments and develops responsive initiatives and work plans including measurable goals and objectives to achieve maximum community participation in health education and screening programs with specific attention to increasing awareness and participation of low income, un-/under-insured, geographically isolated, and other vulnerable populations.
Understands and interprets principles of public health and health equity frameworks to lead, participate in, or advise program design and delivery.
Acts a liaison between Brown University Health and community to promote increased awareness of and participation in various health education and screening programs and to inform quality improvement initiatives across Brown University Health departments and service lines. Reflects awareness of and sensitivity to various cultural, ethnic and racial differences affecting delivery and utilization of health services.
Establishes and maintains effective relationships with community organizations, including but not limited to, area religious organizations, Health Equity Zones, Community Assistance Programs, minority-serving agencies, and community coalitions. Regularly participates in meetings with representatives of these organizations to maintain awareness of evolving needs and to inform an appropriate, coordinated response through collective impact. Develops and maintains effective contacts with various state agencies to solicit support for strategically aligned programs.
In collaboration with others at Brown University Health and in the community, and based on needs assessment, delivers and/or coordinates provision of health education and screening programs such as Safe Sitter, CPR classes, Tar Wars, workshops, conferences, etc. Coordinates speakers, presentations, handouts, marketing materials, etc. to support programs.
Serves as resource to community, provides information about Brown University Health programs and provides written literature and/or qualified contacts as appropriate. Coordinates preparation and distribution of brochures and flyers to educate community on services and programs available.
Evaluates the effectiveness and impact of health programs as it relates to the local community and population health improvement targets. Prepares and maintains various reports and analyses regarding utilization and effectiveness of such programs.
Participates in councils, quality improvement teams and other such committees as required.
Maintains quality assurance, safety, privacy, and environmental and infection control in accordance with established policies, procedures, and objectives.
Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
BASIC KNOWLEDGE:
Equivalent to a bachelor 's degree in public health, social sciences or health care sciences. Knowledge and understanding of community cultural, social and ethnic issues affecting use of health services and system.
Master 's degree in public health, health care administration, social work, or related field preferred.
EXPERIENCE:
Three years progressively responsible experience in public health, social service/community action programs, and/or health care delivery systems.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Employee will be provided with a work space, phone, computer, access to a printer, and all required software.
Employee must have a valid driver 's license, have access to a personal vehicle, and maintain vehicle insurance to travel between system affiliates and community partner organizations.
Employee must be able to lift and maneuver up to 50 pounds of supplies and materials at a time.
