Nurse PractitionerThe Nurse Practitioner provides high-quality, patient-centered care in a clinical setting. This role is responsible for the evaluation, diagnosis, treatment, and follow-up care of patients within the NP scope of practice. The NP supports a collaborative, team-based care model focused on improving patient outcomes and experience.Clinical CarePerform comprehensive patient assessments, including medical history and physical examinationsDevelop and implement evidence-based treatment plansOrder, interpret, and follow up on laboratory tests, imaging, and diagnostic studiesPrescribe medications (including controlled substances) in accordance with California regulationsManage chronic conditions such as diabetes, hypertension, COPD, and CHFEvaluate and treat acute conditions (e.g., colds, flu, infections, minor injuries, rashes)Initiate referrals and coordinate care with specialistsDeliver culturally competent care to diverse patient populationsCollaborate with clinical staff to optimize patient outcomesCare Coordination & Patient ManagementEnsure continuity of care through follow-up visits, chart review, and care coordinationDevelop and manage treatment plans, referrals, and prescriptions within scope of practiceCollaborate with supervising or partnering physicians per standardized proceduresParticipate in interdisciplinary care planningSupport preventative care programs such as Annual Wellness Visits (AWVs) and chronic disease managementMaintain accurate, timely, and compliant documentation in the EMRCompliance & QualityAdhere to established clinical protocols and standardized proceduresMaintain compliance with HIPAA, OSHA, infection control, and regulatory requirementsParticipate in quality improvement initiatives, chart audits, and peer reviewsMeet performance and quality metrics (e.g., HEDIS measures, immunizations, RAF accuracy, AWVs)Patient ExperienceProvide compassionate, respectful, and culturally sensitive careEducate patients on treatment plans, medications, and preventative healthPromote a positive, team-oriented environmentManage time effectively to reduce patient wait timesSupport colleagues during high patient volume to ensure timely care deliveryLeadership & CollaborationServe as a clinical resource for medical assistants, LVNs, and support staffParticipate in staff meetings, training, and workflow improvement initiativesCollaborate with clinical and operational leadership across sites as neededPerform additional duties as assignedRequired QualificationsActive California Nurse Practitioner (NP) licenseNational board certification (e.g., FNP, AGNP)DEA license and furnishing number with Schedule II authority (or eligibility)Current BLS/CPR certificationMinimum 1–2 years of primary care experience preferredPreferred QualificationsBilingual (English/Spanish)Experience with family practice, geriatrics, or senior-focused careFamiliarity with HMO/IPA models and risk adjustment coding (RAF, HCC)Experience in multi-site or high-volume clinical environmentsPhysical & Work Environment RequirementsAbility to stand, walk, bend, and sit for extended periods throughout the dayAbility to lift up to 20–30 lbs as neededWork performed in a clinical environment, including exam rooms and medical officesExposure to moderate noise levels and infectious materialsAbility to safely assist patients with mobility and transfersAdditional RequirementsTo perform this role successfully, an individual must be able to carry out all essential duties effectively.
