Position Summary
The Case Manager promotes and supports adolescents and transitional aged youth who are at risk of developing or have mental health and/or substance abuse issues. Services include assessments, supportive level counseling, advocacy, and link/referral to needed services as indicated in the ISP. Case Manager will link, and coordinate identified client and/or family needs (e.g., medication assisted treatment, housing, employment, etc.). This position is a grant-based position and may come to an end with the ending of grant funding.
Required Education and Experience
- Bachelor's degree from an accredited college/university in an applicable human services field. Or relevant education, which indicates that the individual possesses the required knowledge, skills, and abilities.
- A minimum of one year of relevant mental health, and/or substance abuse work experience with the identified population, or a combination thereof that demonstrates candidate possesses the required knowledge, skills and abilities; and/or an equivalent combination of education, training and experience that provides the individual possesses the required knowledge, skills, and abilities.
Related Knowledge, Skills and Abilities
Services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disabilities, and substance abuse treatment programs. The nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served, including clinical and developmental issues. Different types of assessments, including functional assessments, and their uses in service planning. Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination. The service planning process and major components of a service plan. The use of medications in the care or treatment of the population served. All applicable federal and state laws, state regulations, and local ordinances. Identifying and documenting an individual's needs for resources, services and other supports. Using information from assessments, evaluations, observation, and interviews to develop individual service plans. Identifying services and resources within the community and established service system to meet the individual's needs; and documenting how resources, services, and natural supports, such as family, can be utilized to achieve an individual's personal habilitative/rehabilitative and life goals. Coordinating the provision of services by public and private providers. Work as team members, maintaining effective inter- and intra- agency working relationships. Work independently, performing position duties under general supervision. Engage and sustain ongoing relationships with individuals receiving services. To drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites. To participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed. Willing and available to work modified schedules (weekends, holiday, etc.) as required.
Demonstrate a positive regard for individuals and their families (e.g., allowing risk taking, avoiding stereotypes of developmentally disabled people, respecting individuals' and families' privacy, believing individuals can grow). Delivery of care to clients with diverse values & beliefs, including tailoring service delivery to meet the client’s social, cultural and linguistic needs
