Shift is M-F, 8a-5p. One on-call weekend shift per month. Reports and records any changes in condition or behavior of the patient. Adapts to changes in workload or patient assignments without interference to patient care. Responsible for completion of assignments, maintaining quality work. Establishes good rapport with patients and families, respecting patients rights to privacy and confidentiality of information. Accurately communicates in writing through utilization of appropriate charting methods, and turns in all paperwork on time. Provides support to patients and families in a courteous manner. Performs routine duties without physical supervision. Accurately communicates patients information to the RN Case Manager, LPN and/or Assistant Director of Clinical Services. Assists in maintaining equipment assigned to patient care, and reports malfunctioning equipment to appropriate personnel. Any and all other duties and responsibilities as assigned. QualificationsRequired Skills/Abilities: Excellent verbal and written communication skills. Excellent interpersonal and customer service skills. Excellent organizational skills and attention to detail. Excellent time management skills, with a proven ability to meet deadlines. Strong analytical and problem-solving skills. Ability to prioritize tasks, and to delegate them when appropriate. Ability to function well in a high-paced, and at times, stressful environment. Proficient with Microsoft Office Suite or related software. Education, Licenses, Certifications and Experience: High School Diploma, or GED equivalent. Current Home Health Aide certification. Valid CPR Certification. Certified to pass Home Health Aide Competency Test. 1 year of Home Health Aide or LTCF experience preferred. Physical Requirements: (Subject to Reasonable Accommodation) Ability to speak, hear, and read English. Lift up to 20 lbs. from floor to 36, and from 36 to 64. (Occasional) Sitting.
