Physical Therapist Assistant (PTA) | Part-Time Home Health Service Area: Northside Suburbs of Chicago, Illinois, including Lake Forest, Glenview, Buffalo Grove, Mundelein, and nearby communities. Join the One Home Health Team One Home Health is currently looking for a motivated and caring Physical Therapist Assistant (PTA) to support our home health patients on a part‑time basis. In this position, you'll work closely with a dedicated interdisciplinary team while delivering individualized therapy services in patients' homes. This is an excellent opportunity for a PTA who values autonomy, flexibility, and meaningful one‑on‑one patient care. Primary Responsibilities In this role, you will: Provide prescribed physical therapy treatments and patient education under the direction of the supervising Physical Therapist and according to the physician-approved plan of care. Monitor patient progress and communicate changes in condition, treatment response, or scheduling needs to the appropriate team members. Maintain timely and accurate clinical records that meet agency, state, and federal documentation requirements. Collaborate with nurses, therapists, physicians, and other healthcare professionals to promote positive patient outcomes. Assist with care coordination and participate in case conferences or interdisciplinary team meetings when requested. Follow established infection control practices, safety procedures, and equipment usage guidelines during home visits. Protect patient confidentiality and comply with all HIPAA and privacy standards. Adhere to agency protocols, regulatory requirements, and professional practice standards. Participate in educational programs, staff meetings, and ongoing quality improvement initiatives. Carry out additional duties related to patient care and agency operations as assigned. Required Qualifications Candidates should meet the following requirements: Current, unrestricted Physical Therapist Assistant license in the state of Illinois. Graduate of an accredited PTA educational program recognized by the American Physical Therapy Association (APTA) . Minimum of six months of PTA experience in a healthcare setting preferred. Strong verbal communication, interpersonal, and clinical documentation skills. Reliable transportation with a valid driver's license and current automobile insurance. Ability and willingness to travel throughout the assigned coverage area to provide in-home patient care.
