The Social Worker collaborates closely with the clinic physicians, providers and staff, community health workers, and other healthcare professionals to enhance patient outcomes, reduce readmissions, and improve the overall patient experience. MINIMUM QUALIFICATION(S):
- Master's degree in Social Work (MSW)
- Current Social work licensure in Pennsylvania
- Minimum if two (2) years experience
- Previous experience with Cerner and/or Wellsky
- Certification in case management from a nationally recognized organization.
