Home Health Occupational Therapist

Schedule
Shift: 8-hour days
Weekends: Not required

Assignment Details
Visits per day: Up to 6 patients/day

Requirements
- Minimum 1 year of skilled home health experience required
- OASIS experience required
- Licensing: Both Oregon and Washington licensure required
- Required certification: BCLS

Roles & Responsibilities
- Conduct patient evaluations and develop individualized treatment plans.
- Implement therapeutic interventions to promote patient independence in daily activities.
- Document patient progress and OASIS assessments accurately.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Educate patients and their families on home safety and functional activities.
- Monitor and adjust treatment plans based on patient progress and feedback.

Compliance / Notes
- Guaranteed hours: Client may call off up to one shift per pay period.
- Dress code: Business casual attire is required (scrubs allowed but not mandatory).
- Mileage reimbursement: Provided at IRS standard.
