As a Home Health Medical Social Worker (MSW) , you will provide critical support and resources to patients and their families in the comfort of their own homes. Your Key Responsibilities Include Provide emotional and practical support to patients and their families in their homes Assess patients' needs and develop individualized care plans Link patients with community resources and services Collaborate with interdisciplinary healthcare teams to provide holistic care Maintain accurate and up-to-date patient records Stay current with advancements in home health care and social work practices Must Have Bachelor's or Master's degree in Social Work At least one year of Medical Social Work experience; home health experience preferred Experience in health care, home care or home health care settings Ability to work independently and manage caseload Strong understanding of health care systems and policies Proficiency in electronic medical records (EMR) systems Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication. Skills & Attributes Strong critical thinking skills when working independently in the field. Excellent interpersonal skills to build rapport with diverse patient populations. High level of organization and time management to navigate a daily route. Nice To Have Experience in acute care and working in a home environment. Experience with KanTime EMR.
