Outpatient Registered Nurse - RN

Overview The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care, including assessment, planning, intervention and evaluation for an assigned group of patients. It may involve delegation of tasks to RN, LVN/LPN, and Patient Care Technicians, as well as participation in decision‑making, teaching and quality improvement activities that enhance patient care outcomes and facility operations. Principal Duties and Responsibilities Assist other health care members in providing patient‑specific detailed education regarding adequacy measures such as Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and disease process/access. Welcome assigned patients and inquire about their wellbeing since the last treatment. Evaluate vascular access for patency, perform cannulation, and administer heparin as delegated or as allowed by state law. Obtain pre‑ and post‑treatment vital signs and weight; perform vascular access evaluation before treatment. Initiate dialysis treatment according to prescribed orders, including blood flow (QB) and dialysate flow (QD). Evaluate intradialytic problems and provide interventions as prescribed or directed. Monitor patients’ response to dialysis therapy and obtain vital signs before reinfusion. Discontinue treatment according to established procedures and evaluate patients prior to venous access termination. Obtain hemostasis and apply appropriate dressings; evaluate patient prior to discharge. Record pre‑ and post‑dialysis evaluations, weight, and vital signs with initial identification. Calculate and enter individual dialysis machine programming for ultrafiltration goals, treatment time, and modeling as prescribed. Ensure safety alarms are enabled, including Narrow Venous Limits (NVL) 160. Accurately document vital signs, treatment parameters, and patient responses on the Hemodialysis Treatment Sheet and progress notes. Report significant changes in patient condition to the registered nurse or supervisor. Conduct machine safety checks, report unusual findings, and document interventions. Obtain prescribed laboratory tests and prepare specimens for collection. Clean blood spills immediately with appropriate disinfectant. Organize and efficiently utilize supplies and equipment to prevent wastage. Physical Demands and Working Conditions Provide direct patient care that involves heavy lifting, moving of patients, and assistance with ambulation. Stand for frequent, prolonged periods and bend over regularly. Assist moving machines and equipment up to 200 lbs., and lift chemical or water solutions up to 30 lbs. up to 5 feet. Work in a health care facility with controlled air temperature and moderate noise levels. May be exposed to infectious or contagious diseases/materials. Perform desk and computer work, and interact with patients, staff and physicians. May require travel to training sites or other facilities. Essential functions may be required in other locations with similar physical demands. Supervision Oversee Patient Care Technicians, LPNs, LVNs, and RNs as a Team Leader or Nurse in Charge after meeting criteria. Criteria include successful completion of all FKC education and training requirements; a minimum of 9 months RN experience; and at least 3 months chronic/acute hemodialysis experience as a RN. Education and Licensure Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or successful completion of CPR BLS Certification. Must meet practice requirements in the state of employment. Experience and Required Skills Entry level for RNs with less than 2 years Nephrology Nursing experience.
