LCSW Social Worker

Job Summary:

The Licensed Clinical Social Worker (LCSW) will provide acute care social work services including discharge planning and care coordination across multiple hospital units. The role requires collaboration with interdisciplinary teams to support patient transitions and ensure continuity of care. The LCSW will work in a Level II Trauma Center setting, utilizing Epic EMR systems, and must have acute care experience. Strong communication and case management skills are essential to meet the needs of diverse patient populations. Responsibilities:

• Conduct comprehensive psychosocial assessments for patients in acute care settings.
• Develop and implement discharge plans and coordinate care with healthcare teams.
• Collaborate with interdisciplinary teams to facilitate patient transitions and continuity of care.
• Document all patient interactions and care plans accurately in Epic EMR.
• Provide crisis intervention and counseling services as needed.
• Work across multiple hospital units, including Med Surg, ED, and Long Length of Stay (LLOS) populations.
• Participate in care coordination meetings and communicate effectively with patients, families, and providers.
• Maintain compliance with hospital policies, licensing requirements, and professional standards.

Required Skills & Certifications:

• Master of Social Work (MSW) degree.
• Active Washington State LCSW license.
• Minimum of 1 year acute care social work experience.
• Experience with discharge planning and care coordination.
• Current BLS certification.
• Proficiency with Epic EMR system.
• Ability to work collaboratively in a fast-paced hospital environment.

Preferred Skills & Certifications:

• Experience with AIDA EMR system.
• Diverse case management experience across medical surgical, emergency department, and long length of stay units.
• Previous travel healthcare experience.

Special Considerations:

• Must be willing to work every other weekend.
• Floating between multiple hospital units and campuses (Colby and Pacific) required.
• Strict adherence to RTO (Return to Office) policies as specified at time of submittal.
• Facility prohibits scrub colors that are reddish orange.
• Parking costs are covered by the facility.
• Locals are not accepted; candidate must have a Washington license.

Scheduling:

• Five days per week, 8-hour day shifts.
• Contract length of 13 weeks.
• Guaranteed minimum of 72 hours per two-week pay period.
