Community Health Worker - Outreach & Empowerment
Essentia Health-Graceville Clinic
Essentia Health's West Duluth Clinic is seeking a Community Health Worker to support patients in the Duluth area. You will outreach, enroll eligible patients, and partner with families to set personalized wellbeing goals while connecting them to healthcare and community resources.
