Meadow Creek Post-Acute is a 104-bed Skilled Nursing Facility located in central Paramount, CA. The The ideal candidate has a minimum of 1 year of experience as a CNA working in a skilled nursing facility/subacute unit. This role will be on a full time basis.

Pay Range:

• $19-22/hour Depending on Experience 

Position Overview: 

As a CNA, you will support your assigned residents with routine daily activities and nursing care services in accordance with the resident’s assessment and care plan, and as may be directed by the supervisors.

• Assist residents with Activities of Daily Living (ADLs): bathing, grooming, dressing, toileting, feeding
• Take and record vital signs (blood pressure, temperature, pulse, respirations)
• Help with mobility and safe transfers using gait belts, Hoyer lifts, and other devices
• Maintain a clean, safe and comfortable environment 
• Observe and promptly report any changes in a resident’s physical or emotional condition
• Collaborate with nurses and interdisciplinary team members as needed
• Accurately document care and observations according to facility policy and regulations

Qualifications:

• Highschool diploma or GED     
• Active CNA Certificate in CA 
• BLS/CPR 
• Must have 1 year of experience as a CNA
