Salary Range: $68-$73 per point

Description: We are looking for a Home care Physical Therapist. The Home Care PT evaluates the patient functional needs, creates customized treatment plans, and delivers one-on-one therapy to help individuals recover from surgery, injury, or chronic illness while living at home.

Responsibilities

• Patient Evaluations: Conduct initial assessments, start-of-care visits, and regular re-evaluations to measure functional independence and progress
• Custom Care Plans: Design and update personalized treatment plans under a physician's direction.
• Therapeutic Interventions: Provide gait and balance training, muscle strengthening, pain management, and instruction on using assistive devices (walkers,canes).
• Home Safety Assessments: Inspect the patient's living environment for safety hazards and recommend necessary modifications or accessibility tools.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program
• Minimum of 1 year of clinical or home health care experience preferred
• Valid driver's license, reliable personal automobile, active auto insurance, and willingness to drive to patients homes
• Effective written and verbal communication
• Ability to stand, bend, lift and transfer patients (frequently lifting up to 50-100 lbs)
• US Citizen or Green Card holder
• Current, active, and unrestricted Physical Therapist state license
