Responsibilities Participate in planning and coordinating total patient care with the RN and physician, following the nursing care plan. Accurately report and record patient conditions and care, noting signs of change. Prepare clinical notes and summaries. Assist patients with daily activities and teach self-care techniques. Maintain a safe environment for patients. Assist physicians and RNs in complex nursing situations. Follow universal precautions during patient care. Follow emergency procedures in case of incidents or significant patient condition changes. Trach care is a MUST. Qualifications Minimum 1 year of experience as an LPN in a home care setting. Current state LPN license. Graduate of an accredited practical nursing program. Proof of current CPR certification. Ability to work a flexible schedule and travel locally. Excellent interpersonal skills. Valid driver’s license and reliable transportation.
