Wraparound Care Coordinator The Wraparound Care Coordinator works within the Agency’s High‑Fidelity Wraparound program, serving youth ages 12‑20 with a diagnosed mental health disorder or developmental delay who are involved with the Juvenile Justice system. The role provides home‑based and community‑based case management services to youth and families, supporting the development of individualized goals and the maintenance of families in their homes and communities. Responsibilities Deliver intensive case management services, collaborating with a multidisciplinary team that includes both professionals and natural supports. Develop and implement individualized goals for youth and families in partnership with the Wraparound team. Attend monthly staff meetings and participate in weekly supervision with the assigned supervisor. Maintain accurate and timely documentation and paperwork, including case notes and reporting requirements. Provide flexibility in scheduling, including evenings, to ensure continuity of care and meet client needs. Conduct family visits, which may involve physical activities such as traveling, climbing stairs, and lifting up to 25 pounds. Qualifications Bachelor’s degree in Social Work or a related field. Knowledge of serious and persistent mental illness, substance abuse, and trauma. Minimum of two years of experience working with families and children, preferably within the mental health system. Strong ability to work independently and collaboratively. Willingness to work flexible hours, including evenings, and to travel within the program service area. Physical ability to lift and carry up to 25 pounds, stand, walk, kneel, and lift small children or materials. Proficiency with Microsoft Office and email. Successful completion of a criminal background check, child protective service check, and sex offender check, and not be on state or federal suspension and disbarment lists.
