Home Health Occupational Therapist (OT)
Location: Sacramento

- Deliver occupational therapy to patients facing challenges in daily tasks due to illness or injury
- Collaborate with a home health team, which may include RNs, LPNs, CNAs, HHAs, PTs, and SLPs, to focus on the patient’s care plan and personal objectives
- Evaluate patients, identify deficits, set therapy goals, and document progress
- Guide patients through exercises aimed at enhancing fine motor skills and coordination
- Recommend adaptive tools, such as grab bars and shower chairs, to provide additional support
- Assess fall risks and implement strategies to enhance home safety
- Educate patients and their families about the care plan, exercises, goals, and self-care techniques
- Graduation from an accredited Occupational Therapy Program and possession of an active OT license in California
- At least 1 year of experience in occupational therapy, preferably in home healthcare settings
- Current CPR certification and any additional relevant certifications
- Familiarity with state and federal home health regulations
- Strong clinical judgment, excellent interpersonal skills, resourcefulness, and compassion
