Through innovative programming and strategic partnerships, WHA provides services in Community Wellness, Reentry and Adult Services, Workforce Development, Youth Innovation, and Economic Development. WHA works to improve the health, well-being, and quality of life of individuals, families, and communities by creating opportunities that promote long-term stability and self-sufficiency. POSITION SUMMARY The SC2 Workforce Case Manager is responsible for supporting participants from intake and enrollment through program completion within the Scaling Community Violence Intervention (SC2) Workforce and Job Development Program. The position provides participant-centered case management, coordinates supportive services and referrals, maintains accurate program documentation, and helps participants address barriers that may affect engagement, workforce readiness, and employment success. The Case Manager works collaboratively with the Program Manager and Employment Specialist in a cohort-based workforce development environment. This role requires professionalism, sound judgment, strong organization, attention to detail, effective communication, and the ability to manage responsibilities independently while contributing to a collaborative team. This position is subject to continued grant funding availability, program performance requirements, and organizational needs. ESSENTIAL DUTIES AND RESPONSIBILITIES Participant Intake, Enrollment & Assessment Conduct participant intake, enrollment, orientation, and related onboarding activities. Receive and process participant referrals in accordance with program procedures. Complete participant assessments and all required enrollment and program documentation. Create, organize, and maintain complete participant files from intake through program completion. Identify participant strengths, goals, barriers, service needs, and other factors that may affect successful program participation. Case Management & Participant Support Provide ongoing case management services and individualized participant support throughout program participation. Maintain regular communication with assigned participants to promote engagement, attendance, accountability, and successful completion of program activities. Monitor participant attendance, engagement, progress, and accountability throughout the cohort. Identify barriers to participation or employment and coordinate appropriate supportive services, internal resources, and community referrals. Follow up on referrals and participant needs to support continuity of services and progress toward program goals. Maintain professional boundaries, protect participant confidentiality, and provide culturally responsive, participant-centered service. Program Operations & Cohort Support Support the daily operations of the cohort-based workforce development program in collaboration with the Program Manager and Employment Specialist. Assist with participant orientation, scheduling, program activities, workshops, and other cohort needs as assigned. Collaborate with the Program Manager and Employment Specialist to promote participant success, employment readiness, successful transitions to employment, and achievement of program outcomes. Participate in case reviews, staff meetings, supervision sessions, trainings, departmental initiatives, and organizational meetings as assigned. Documentation, Data Management & Compliance Maintain thorough, accurate, complete, and timely case notes, attendance records, assessments, participant contacts, referrals, service documentation, and other required records. Enter and maintain participant information within multiple required data management systems and ensure records are consistent and up to date. Collect and maintain documentation needed for performance tracking, reporting, monitoring, audits, and grant compliance. Process participant stipend documentation and submit complete and accurate paperwork for payroll processing within established timelines. Ensure participant files and program records comply with organizational policies, program requirements, grant guidelines, documentation standards, and confidentiality requirements. Collaboration, Community Engagement & Representation Represent Westside Health Authority professionally and build positive working relationships with participants, staff, outreach partners, employers, community organizations, and other stakeholders. Collaborate with internal departments and external partners to coordinate participant services and support successful program outcomes. Support participant recruitment, outreach activities, departmental initiatives, special projects, and program events as assigned. Attend community, workforce development, or program-related meetings and activities as required. QUALIFICATIONS Education & Experience High School Diploma or GED required. Experience in workforce development, case management, intake, human services, customer service, community-based programming, violence intervention, or a related field preferred. Experience working with individuals who may face barriers to employment or participation in community-based services preferred. Demonstrated ability to establish rapport, build trust, and maintain professional working relationships with diverse populations. KNOWLEDGE, SKILLS, AND ABILITIES Proficiency in Microsoft Word, Excel, Outlook, email, and general office technology. Ability to accurately enter, maintain, and reconcile participant information across multiple data management systems. Demonstrated attention to detail and ability to maintain thorough, accurate, and timely documentation. Excellent verbal, written, and interpersonal communication skills. Strong organizational and time management skills with the ability to manage multiple priorities and meet established deadlines. Ability to work independently while contributing effectively within a collaborative team environment. Ability to adapt to changing priorities and program needs in a fast-paced environment. Ability to maintain confidentiality and appropriately handle sensitive participant and program information. WORK ENVIRONMENT This position operates in a professional office, classroom, community, and program-based environment and requires regular interaction with participants, staff, community partners, employers, and other stakeholders. Duties may require local travel throughout Chicago and surrounding communities and occasional evening or weekend participation in program activities, outreach events, meetings, or community engagement efforts. PERFORMANCE EXPECTATIONS Maintain accurate, complete, and timely participant documentation and case records. Support participant attendance, engagement, accountability, and successful completion of program activities. Coordinate participant services and referrals in a timely and professional manner. Demonstrate reliability, professionalism, sound judgment, effective communication, and teamwork. Meet applicable program, grant, documentation, and performance requirements. Support the mission, vision, values, and goals of Westside Health Authority. Salary: $51,500.00 annually. Benefits: Eligible for Westside Health Authority employee benefits in accordance with applicable plan terms and organizational policy. Schedule: Full-time; schedule may include occasional evenings or weekends based on program activities, meetings, or community events. Grant Funding: Continued employment is contingent upon the availability of grant funding and applicable program and organizational requirements.
