POSITION OVERVIEW…

Reporting to the Nursing Director, the Field RN has a critical leadership role overseeing the entire clinical services operation for LGA. This role combines clinical oversight, assessment and support with a commitment to regulatory compliance and standardized care across all LGA facilities. Through your leadership skills, you will deliver consistent, high-quality health services and ensure that clinical standards are met organization-wide by partnering with external healthcare providers and directly managing a team of internal nurses and non-clinical care team staff.

YOU WILL BE RESPONSIBLE FOR…

Clinical Oversight of Patient Care: Provide oversight regarding all aspects of coordination of patient care delivery. 

• Prospective Client Reviews: Assess clinical care needs for prospective clients, including Health Risk Screening Tool (HRST) review and participation in 30-minute meet & greets as needed. 
• Healthcare and Care Plan Development: Write and review healthcare plans for all clients. Ensure the development, implementation, and updates are timely and accurate for all clients including new clients and those with changes in conditions. Ensure that client needs are continually assessed.
• HRST Reviews: Conduct clinical reviews 90 days before each client’s birthday or upon significant changes (e.g., new diagnosis, new medications).
• Medication Authentication: Verify monthly scheduled and PRN medications for clients across multiple locations.
• Clinical Nursing Assessments & Reports : Perform, document and/or approve assessments for host, group and private home care clients. Conduct on-site nursing assessments for clients receiving daily LPN care when needed. Approve daily LPN assessments and provide weekly feedback. Manage monthly RN oversight reports for clients receiving RN services. Perform annual RN assessments and healthcare plan reviews as needed.

Clinical Leadership, Training & Compliance: Supervise day-to-day clinical operations and ensure services follow professional standards that are in compliance with state and federal regulatory requirements. 

• Support and Consultation: Provide clinical support to the staff and be available to answer all clinical questions including PRN administration-related topics. 
• TB Testing: Administer TB tests for candidates and new hires. 
• Trainer and Interviewer: Act as a Clinical Academy trainer for direct support employees and serve as a functional interviewer for potential hires. 
• Clinical Compliance Lead/RN DDP: Conduct and submit clinical compliance audits and participate in the Compliance Steering Committee to ensure adherence to regulatory and organizational standards.  
  

Desired Experience

• 2-3 years of RN experience supporting adults with behavioral and or intellectual disabilities in one or more of the following settings: ICU, ER, community health, home health, long term or psychiatric care facilities.  
• Proven clinical oversight and strategic leadership experience.
• Ability to inspire cross-functional teams to deliver exceptional client care.
• In-depth knowledge of DBHDD and/or DCH regulations and experience driving compliance-related requirements across clinical and support staff teams.
• Current CPR certification.
• A valid driver’s license, auto insurance and reliable transportation.
• Nursing Degree from an accredited/approved school of nursing program.
• Current and unrestricted Registered Nurse (RN) licensure.
  
• Experience with Medicaid/Medicare Waiver programs.

PREFERRED EXPERIENCE …

• Specific experience in the Therap EMR tool.
• Specific experience in home health care.
  

This hybrid position is based in Riverdale and requires virtual, onsite, and fieldwork travel across all LGA clinical care facilities.
