Job Title: Outpatient Physical Therapist

Description:
This position involves providing patient assessments.
The therapist will develop and implement individual patient treatment programs.
Therapeutic interventions will be performed utilizing current and effective methodologies.
Effective documentation of treatments and patient interactions is required.
Evaluations must be completed according to department protocol within 48 hours of initiation.
Appropriate goals must be set relative to the patient's prior level of function.
The treatment plan should be relevant to the identified problems of the patient.
Patient and family participation in the treatment plan and goal setting is essential.
Ongoing assessment of treatment effectiveness is required.
Goals and treatments should be modified as appropriate in relation to progress.
Selection of appropriate treatment interventions must be demonstrated.
Patient safety must be addressed, with follow-up recommended as necessary.
Education for patients and families should address comprehensive aspects of care.
A variety of teaching methods should be utilized, appropriate to the level of understanding.
Education must be initiated as soon as the patient or family is capable of learning.
Documentation should comply with department policies and be completed in a timely manner.
Documentation must be concise, legible, and use correct grammar and medical terminology.
Effective communication with the healthcare team and MD is vital.
Participation in team conferences is expected to enhance collaboration.
Feedback from MD and coworkers to improve teamwork is encouraged.
Supervision and delegation to support staff and students is a part of the role.
Time management should comply with department policy for productivity.
The therapist will initiate and complete projects within established timeframes.
Other related duties may be assigned or requested in a professional manner.
