RN - Case Manager

RN Case Management/Discharge PlanningWe are seeking a highly experienced Registered Nurse specializing in Case Management and Discharge Planning. The ideal candidate will have a strong background in acute inpatient care, specifically within a large, busy hospital setting. This role requires a proactive approach to patient management and effective collaboration with healthcare teams.ResponsibilitiesManage acute inpatient case management and discharge planning for a diverse patient population.Collaborate with social work teams to assist in complex patient placements.Work alongside healthcare professionals to develop and implement effective discharge plans.Utilize strong computer skills and proficiency in EPIC charting for documentation and communication purposes.Consult with respiratory therapy and pharmacy regarding discharge planning as needed.Participate in general facility orientation and department-specific training (2 weeks).Handle an average caseload of 15-20 patients daily.QualificationsMinimum of 2 years of acute inpatient medical case management experience.At least 1 year of recent experience in a large, busy hospital emergency department (500+ beds, Level 1 or Academic Medical Center).Recent discharge planning experience within the last two years, covering a variety of medical diagnoses.Experience in telephonic and utilization management/review is not applicable for this role.Previous travel experience in a Level 1 facility within the last 3 years.Must be BLS certified.Ability to work efficiently and adapt quickly with minimal training.Work EnvironmentThis role involves collaboration with a dedicated social work team, including 15 full-time and 17 part-time social workers. The team will work across various inpatient units, ensuring comprehensive support and care for all patients.Additional RequirementsCompliance with flu vaccine requirements; medical exemptions must be documented.Fingerprinting requirements can be processed remotely or in person prior to starting.Dress code includes any color scrubs or business professional attire.LocationThis position is located in a dynamic healthcare environment in Mississippi.
