This is an on-site position located at Newton Wellesley Hospital.Job Responsibilities:The role of Weekend Nurse Liaison provides information about hospice services to patients, families, and hospital-based staff, conducts clinical assessments, coordinates hospital discharge plans, and maintains professional relationships within hospitals, with a focus on the assigned hospital(s).Schedule and Weekly Hours:This is a flexible role offering between 20 hours (part-time) and 40 hours (full-time), based on the candidate's availability.
