This role offers manageable patient panels and robust care team support, allowing physicians to practice medicine with ample time and purpose. Practice Info * Value-based, full-risk Medicare Advantage model * Patient panel capped at ~450 patients * Focus on prevention, care coordination, and behavior change * Dedicated care teams (MA, Care Coordinator, Care Facilitator) * Emphasis on quality, patient experience, and physician wellness Responsibilities * Provide comprehensive primary care to older adult patients * Maintain full accountability for clinical outcomes and patient experience * Serve as a health coach, guiding lifestyle and long-term health improvement * Coordinate specialty and hospital care while staying actively involved * Lead and collaborate with an interdisciplinary care team * Supervise and remain accountable for Advanced Practice Providers (APPs) * Partner with medical and operational leadership to meet quality and cost goals Compensation * Salary: $250,000 - $306,714 (based on experience) Benefits * Comprehensive benefits package * Paid time off and CME * Long-term career growth and leadership opportunities * Strong work-life balance Shift & Schedule * Predictable schedule * Appointment utilization (18-21 slots per day) Requirements * MD or DO required * Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics * Active or eligible state medical license * Active DEA * Commitment to value-based, team-oriented care
