• Opportunities: Full-time, part-time, and PRN (as needed), based on available patient assignments.
• Work setting: In-person care in patients’ homes.
• Location: Appling, GA, and surrounding service areas; assignment locations vary.

• Schedule: Monday, Wednesday, Thursday, Friday afternoons: 4:00 PM – 8:00 PM
• Approximately 16 hours per week
• Qualifications & Skills
• Active, unrestricted Georgia RN or LPN license
• Experience with G Tube feedings is required
• Pediatric home health experience preferred
• Cerebral Pasly, ADL Assistance, experience preferred
  Pay: LPN Annual Salary: $65,000–$75,000

Gentle Care Home Services is seeking compassionate and dependable Licensed Practical Nurses (LPNs) interested in providing personalized, one-on-one nursing care to children in their homes. Our patient needs and schedules vary, allowing our recruiting team to help identify available cases that may be a good match for your clinical experience and availability.

Responsibilities

• Provide one-on-one nursing care according to the patient's plan of care.
• Monitor and report changes in the patient's condition.
• Administer medications and prescribed treatments.
• Provide assigned skilled nursing care, which may include G-tube, tracheostomy, ventilator, and seizure care, according to the patient’s care plan, your scope of practice, and verified competencies.
• Assist with activities of daily living as appropriate.
• Complete accurate and timely documentation.

Requirements

• Active, unrestricted Georgia Licensed Practical Nurse (LPN) license.
• Current CPR certification.
• Ability to meet required background and health screening requirements.
• Pediatric or home health experience preferred.

• Required clinical skills vary by assignment. Our clinical team will review your experience and competencies to n determine an appropriate patient match.
