Home Health Licensed Practical Nurse

Overview Pride Health is hiring a Licensed Practical Nurse to support our client’s medical facility in Atlanta, GA 30328 . This is a 3+ month contract assignment (convertible to permanent) and a great way to start working with a top-tier healthcare organization! Specific Duties Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable) Actively participates in the agency's care coordination process including timely reporting patients' needs and changes in condition, attending patient case conference, communicating effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable) Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client Qualifications Minimum of 1 year of preferred experience High school graduate or GED equivalent Current and unrestricted LPN licensure in state of practice Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation Current BLS certification (through an approved American Heart Association Training Centre or the Military Training Network) at the time of hire or within 30 days of hire Requirements Current CPR certification or ability to complete within 90 days of hire Ability to manage multiple tasks simultaneously Able to work independently Good communication, writing, and organizational skills Ability to work flexible hours as required to meet identified client needs #PHCUHGClinical $250 Pay rate will be based on education, experience, and healthcare credentials.
