Your Specific Responsibilities:

The incumbent, an employee of the Citrus County Health Department, represents the agency and its mission and values. The incumbent is part of the global public health organization and is expected to promote cooperation, courtesy and teamwork, in a diverse environment. This position is responsible for implementing the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) at different clinic sites throughout the county as directed by the Nutrition Program Director.

Provide nutrition assessment, nutrition education, breastfeeding education and counseling, including high risk care plans, when appropriate, to clients enrolled in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC). This position requires independent judgement based upon information found in the DHM 150-24 Policy and Procedure Manual for the WIC Program and the FL-WiSE Clinic Manual, FL-Wise Training Manual and Modules, including WiSE Notes, FL-WiSE Help Sheets.

Performs and assess anthropometric, medical, and dietary data to determine client's eligibility for the WIC program in accordance with policies of the State WIC program and regulations stated in DHM-150-24. Perform anthropometric and hemoglobin measurements for clients during WIC certification, recertification, mid-certification, and follow ups when needed. Certifies and recertifies clients for the WIC program, including nutrition assessment and assignment of nutrition risk factors base upon procedures in the DHM 150-24 Policy and Procedure Manual. Prescribes and makes changes in WIC food packages for clients.

Provides nutrition counseling to WIC participants and promotes nutrition/education goal setting pertinent to the needs of the client.

Document services in FL WISE using Subjective, Objective, Assessment and Plan (SOAP) charting method. Develop, monitor, evaluate and revise nutrition care plans for individual clients at high nutritional risk. Monitor and follow protocols for nutrition assessment and follow up for high risk WIC participants. Develops care plans and follows up with medically high-risk clients in providing medical nutrition therapy counseling according to guidelines set up by Academy of Nutrition and Dietetics and as stated in DHM-150-24. Discusses client's progress with other professionals. Assess medical documentation forms to determine need for special formula or non-contract formula following the guidelines stated in DHM-150-24. Provide referral to other health care and social service programs. Coordinate services with other CHD programs, including immunizations, primary care, Healthy Start, etc. Participate in breastfeeding promotion (via one-on-one education or outreach), breastfeeding training and activities (including World Breastfeeding Week), and participate in WIC outreach events, as needed.

Participate in the preparation and selection of information materials and presentations, including secondary nutrition education resources, bulletin boards, handouts, employee wellness, etc. for use in the WIC nutrition program and for various community groups.

Ensures that nutrition or physical activity DVD's are played in the lobby for WIC families. Instructs WIC Health Support Specialists on the quarterly nutrition education kits.

Prepares lesson plans and conducts classes for WIC participants related to nutrition factors affecting infants, children and pregnant and lactating women.

Conducts Healthy Habits individual appointments or small group education for families with overweight or obese children. This will include an initial appointment, along with at least one individual follow-up appointment. Individual goal setting with families is completed using goal setting tool in FL-WiSE.

Perform quality assurance chart reviews for low risk and high-risk clients and other quality assurance forms as required.

Collects and evaluates information from the WIC records, which can be used for quality improvement purposes. Participate periodically in WIC and other health surveys as required.

Provide technical assistance on program policies and current nutrition information to CHD staff. Attends required CHD meetings, including regularly scheduled staff meetings. Requires travel in various clinic locations and other community partners in Citrus County throughout the day.

Complete all required forms and reports such as Employee Activity Reports (EARs), Missing Bloodwork, High Risk Contacts, attendance and leave records, travel expenses, etc. by the required deadlines.

Perform other related duties as assigned. This position is responsible for the secure collection and/ or access to records containing social security numbers.

Required Knowledge, Skills, and Abilities:

Knowledge of the current science and practice of food and nutrition, dietetics or food service. Knowledge of normal nutrition principles and food needs throughout life cycle. Knowledge of meal planning, food budgeting and purchasing. Ability to provide nutrition education to others in groups or individual instruction. Ability to determine nutritional needs and develop nutrition education plans for individual who are low, high or medical high nutritional risk. Ability to educate individuals and families about food and nutritional needs. Ability to participate in education or in-service training programs through teaching classes, demonstrating food practices, giving talks, and developing or selecting educational materials. Ability to plan, organize and coordinate work assignments. Ability to establish and maintain effective working relationships with others. Ability to maintain records, communicate, maintain effective working relations and good rapport. Ability to understand and apply applicable rules, regulations, policies and procedures.

Qualifications:

Minimum - This occupation requires that the incumbents meet the requirements outlined in Chapter 468, Part X, Florida Statutes, or be eligible to practice dietetics in accordance with Chapter 64B8-40 to 45, Florida Administrative Code and/or Registration as a dietitian as defined by the Academy of Nutrition and Dietetics.

Preferred - 1 year or more of WIC Competent Professional Authority experience.
