Casework Associate - Community Health Advocate

The City of Cincinnati is seeking a multi-functional health program assistant who performs clinical, administrative, and clerical tasks under a Team Leader. You will conduct client interviews, assess needs, maintain records, and coordinate referrals and follow-ups while supporting a team-based approach to care.
