Overview RISE Early Intervention and Therapy for Developmental Disabilities, LLC provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child’s strengths and to enhance and promote the child’s growth, development and learning skills in their natural environment. Location: Phoenix area (including Mesa, Gilbert, and surrounding East Valley cities). Responsibilities Provide evaluations, assessments, and ongoing physical therapy for children birth to three years of age. Work as part of a multidisciplinary core team to provide screenings, assessments, planning, and ongoing services in collaboration with the child’s family, medical providers, and other community representatives. Participate in Core Team meetings, one-on-one family sessions, and Individual Family Service Plan meetings. Deliver services in the child’s natural environment (home, daycare, park, etc.) in the context of the family’s routines and activities. Maintain a flexible schedule, including evenings or weekends as needed, and travel to attend meetings and provide services in and around the Phoenix/East Valley cities. Independent contractor position; services provided in a family-centered, natural-environment setting. Qualifications Master’s Degree in Physical Therapy. Licensure as a Physical Therapist (state requirements applicable). In-home and clinical setting physical therapy experience; pediatric experience preferred. Completion of AzEIP standards of practice for Early Interventionists within three years of start date; demonstrable knowledge of AzEIP Policies and Procedures and Team-Based Model. Meet all prescribed professional requirements and rules of professional conduct for Physical Therapists, including applicable licensure. Background clearance required. Continuing Education The practitioner shall complete the continuing education units required to maintain professional credentialing when employed as a licensed, registered, and certified Physical Therapist. Services are supported under contract with the Arizona Early Intervention Program (AzEIP) participating agency. Compensation and Employment Details RISE Early Intervention offers competitive pay depending on experience. This is an independent contractor position serving clients in Arizona, with services supported under contract with AzEIP.
