Job Summary:
Seeking an experienced Registered Nurse - Case Manager to provide specialized acute care management in a hospital setting. The role involves coordinating patient care, ensuring compliance with clinical guidelines, and collaborating with multidisciplinary teams to optimize patient outcomes.

Responsibilities:

• Manage and coordinate patient care plans for hospitalized individuals, ensuring adherence to clinical protocols.
• Utilize Meditech, Midas, and Interqual systems to document and evaluate patient progress.
• Educate patients and their families about treatment plans, discharge procedures, and follow-up care.
• Collaborate with physicians, social workers, and other healthcare professionals to deliver comprehensive care.
• Monitor patient statuses, identify care needs, and adjust management strategies accordingly.
• Ensure regulatory compliance and maintain accurate documentation for all case management activities.

Qualifications:

• Hold an active RN license with recent acute care hospital experience in case management.
• Demonstrate proficiency with Meditech, Midas, and Interqual systems.
• Possess strong clinical assessment skills and knowledge of healthcare regulations.
• Exhibit excellent communication and collaboration skills within multidisciplinary teams.
• Capable of working every other weekend and flexible hours as required.
